Core Responsibilities
- Lead a team of Medical Assistants in collaboration with Clinical and Operational management
- Oversee Chronic Care Management (CCM), Physical and Rehabilitation Medicine (PRM), Behavioral Health Integration (BHI), and other POD programs
- Manage Medical Assistant documentation, workflow, and performance metrics
- Monitor patient vital signs and address alerts or concerns promptly
- Answer calls, respond to inquiries, and triage appropriately ☎️
- Assist in developing and implementing personalized care plans
- Respond to urgent care situations as needed
- Perform administrative duties efficiently
- Screen patients and facilitate referrals to other clinical services
- Ensure timely completion of clinical requests and follow-ups
- Conduct documentation audits and reviews
- Maintain comprehensive medical records of all patient interactions 📂
- Perform additional duties as required
Work Details
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Location: On-site in Brooklyn, NY 11218 📍
Required Qualifications
- Graduation from an accredited Licensed Practical Nursing (LPN) program
- High School Diploma or equivalent (GED)
- Current LPN licensure and registration in New York State
- Excellent communication, interpersonal, organizational, and time management skills 🗂️
- Ability to work both independently and collaboratively
- Proficiency with EMR systems and documentation
- Compassionate, patient-focused attitude
- Knowledge of state healthcare requirements and policies
- Chinese fluency is required
Preferred Qualifications
- Experience supervising or leading Medical Assistants
- Additional licensure as an LPN in Florida State
Compensation & Benefits 💼
Pay: $32.00 – $40.00 per hour
