Join Our Team as a Bilingual LPN/RN Supporting Patient-Centered Wellness Care 🚑

We are looking for a compassionate, organized, and tech-savvy LPN or RN fluent in Russian and English to empower our Nurse-Led Annual Wellness Visit (AWV) Program, focusing on preventive care, chronic disease support, and telehealth services.

Primary Duties & Responsibilities

• Conduct Medicare AWV preparation and nurse-led patient assessments/screenings under provider supervision
• Engage patients via phone and telehealth platforms to complete health risk assessments, screenings, and preventive reviews
• Document accurately within multiple EMR/EHR systems, ensuring compliance with workflows and policies
• Manage various technology systems simultaneously, including EMR/EHR, telehealth platforms, payer portals, and scheduling tools
• Assist providers in identifying and closing preventative care gaps
• Support chronic disease management care coordination efforts
• Educate patients on preventive services, wellness goals, medication adherence, and follow-up care
• Escalate clinical concerns appropriately to supervising providers or clinical leadership
• Maintain compliance with HIPAA, CMS guidelines, and documentation standards
• Help create care plans, protocols, and standing orders within nursing scope of practice
• Coordinate scheduling and follow-up appointments
• Collaborate with interdisciplinary teams to enhance patient outcomes and engagement
• Communicate professionally with patients, caregivers, providers, and staff

Preferred Qualifications

• Active LPN or RN license in good standing
• Fluent in Russian and English (spoken and written) 🇷🇺🇺🇸
• Experience with Medicare AWV is a plus
• Strong knowledge of EMR/EHR systems and telehealth workflows
• Ability to multitask and navigate multiple systems efficiently
• Excellent communication and patient engagement skills
• Comfortable in a fast-paced remote or hybrid care setting
• Strong organizational and documentation capabilities

Compensation & Benefits 💼

Pay: $28.00 - $60.00 per hour
