Position Overview
The Life Enhancement Liaison/Assistant serves as a vital link between the clinical nursing team and the life enhancement department. This role involves providing life enhancement education to peers, implementing engagement programs within households, offering one-to-one engagement to patients, and promoting person-centered care. In collaboration with nursing and life enhancement teams, the Liaison educates PCAβs on engagement strategies for diverse patient groups and encourages the integration of these practices into small household programs. This position also supports community-wide events, working collaboratively to ensure a high quality of life and wellness for residents. Additionally, the Liaison offers support and guidance to the interdisciplinary team to meet the comprehensive needs of patients, families, and staff, transforming every interaction into a positive and meaningful experience. Participation in self-managed work teams ensures household activities align with the Hebrew Rehabilitation Centerβs mission, vision, and care model.

Position Responsibilities
- Provide ongoing education to household nursing staff to enhance patient engagement and neighborhood programs π
- Act as a liaison between clinical nursing and life enhancement teams to develop efficient policies and procedures π€
- Conduct in-services for staff at departmental, floor, or center-wide levels to highlight the value of life enhancement programs π’
- Assess patient needs to determine the requirement for one-to-one engagement π§βπ€βπ§
- Support peers and PCAs in learning and developing household engagement opportunities π
- Develop and implement therapeutic programs tailored to physical, cognitive, psychosocial, spiritual, and sensory needs of residents π
- Provide direct patient care collaboratively with the interdisciplinary team π
- Document patient progress through notes and reports π
- Coordinate center-wide activities and assist volunteer guides with program support π
- Participate in process improvement initiatives, offering ideas to enhance systems and processes π‘
- Ensure residents are informed of activities and manage transport to and from programs collaboratively π
- Maintain departmental cleanliness and professionalism π§Ή
- Enhance personal knowledge through training, conferences, and workshops, applying new insights π
- Support staff with documentation and perform audit tasks as needed π
- Assist Nurse Managers with projects and unit-specific needs π₯
- Hold regular meetings with supervisors to review unit community life and progress toward goals π
- Perform other duties as assigned to support the departmentβs success π§
Qualifications
- Current Certified Nursing Assistant (CNA) certification or enrolled in a nursing program with clinical experience π
- Experience leading patient group programs in long-term care facilities π―
- Excellent communication and interpersonal skills π¬
- Computer literacy, including experience with databases, Windows, Word, and Excel π»

Physical Requirements
- Push wheelchairs up to 50 lbs on flat surfaces and ramps π¦½

- Assist with patient transfers and transport π§³
- Lifting supplies and equipment up to 50 lbs βοΈ
- Reaching, stooping, squatting, bending, kneeling, and crouching as needed π§ββοΈ
