Position Overview

The Care Partner carries out Activities of Daily Life services for residents according to their individualized service plans within our community. Key responsibilities include providing personal care, communicating with medical professionals and families, and participating in meal services. All duties are performed in full regulatory compliance.

Key Responsibilities

• Follow each resident’s service plan in a friendly, respectful, and supportive manner. Accurately document any observed changes in condition in compliance with laws and policies.
• Contribute to the Continuous Quality Improvement Program, ensuring high-quality, individualized services. Report issues promptly for correction.
• Adhere to all applicable company policies, as well as local, state, and federal regulations, including HIPAA, OSHA, and Blood Borne Pathogen Guidelines.
• Maintain safety standards by working cautiously and reporting unsafe actions immediately. Ensure compliance with Safety Committee Standards.
• Foster teamwork, assist colleagues beyond your immediate responsibilities, and positively impact residents and visitors.
• Coordinate and communicate effectively with fellow Care Partners regarding resident needs.
• Perform additional duties as assigned or necessary.

Required Skills & Experience

• High School Diploma or equivalent
• At least one year of experience in caregiving or a related role
• Proficiency with Microsoft Word and Excel (preferred)
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification (preferred)
• Excellent verbal and written communication skills
• Ability to communicate effectively in person and over the phone
