Assessment Nurse 🩺
Join our team as an Assessment Nurse and play a vital role in conducting field assessments to determine members' eligibility and service needs within downstate New York. This position ensures efficient care coordination, compliance with Department of Health (DOH) regulations, and supports community-based health initiatives.
Roles and Responsibilities
- Conduct home and virtual assessments: Perform evaluations of prospective and current members using the Uniform Assessment System (UAS) to determine enrollment and ongoing eligibility.
- Assessment in various settings: When necessary, conduct assessments at skilled nursing facilities or remotely.
- Service needs evaluation: Complete assessments to identify service requirements and recommend necessary adjustments based on input from the Care Management Team (CMT) and family or caregivers.
- Review medical documentation: Analyze previous nursing assessments and medical notes to update care plans or assist with nursing home admissions.
- In-person care management: Support person-centered service planning during face-to-face visits.
- Member and environment liaison: Assess home environments, psychosocial status, and facilitate communication between members and the CMT.
- Feedback and quality assurance: Provide insights to home care agencies and address issues related to care providers’ performance.
- Community-based training: Educate frail populations on self-care techniques and prevention strategies through grassroots efforts.
- Enrollment clarity: Ensure members and significant others understand and agree with enrollment in managed long-term care plans.
- Additional duties: Perform any other tasks as assigned to support program operations.
Minimum Qualifications 🎓
- New York State RN license
- Ability to travel within downstate New York, including the 5 boroughs, Long Island, Rockland, and Westchester
- For Pediatric Positions Only: 1 year of pediatric clinical experience or work with families and child-serving systems, including child welfare or medically fragile/developmentally disabled populations
Preferred Qualifications 🌟
- Experience in field-based assessments, planning, or evaluations in home or facility settings (e.g., CHHA visits)
- Background in geriatrics, home care, discharge planning, case management, or medical-surgical nursing
- Proficiency with electronic health information systems, especially UAS
- Valid NY driver’s license
- Knowledge of health insurance, Medicaid, Medicare, and MLTCP
- Experience working with frail or elderly populations; skill in creating and negotiating care plans
- Strong time management, critical/creative thinking, communication, and problem-solving abilities
- Language skills in Mandarin and Cantonese (preferred)
- Ability to handle heavy caseloads effectively
- Intermediate skills in Microsoft Word, Excel, and Outlook

Salary Range 💰
Greater New York City Area (NY, NJ, CT residents): $81,100 - $117,470
All Other Locations (within approved areas): $71,600 - $106,505
Your compensation will depend on your experience, education, licenses, and certifications.
