Key Responsibilities

• Coordinate daily admissions from assigned hospitals
• Gather information and conduct on-site evaluations for our short stay program
• Establish and maintain positive relationships with hospital Case Managers, Discharge Planners, and Social Workers

Qualifications & Requirements

• ✔️ Valid driver’s license
• ✔️ 1-2 years of prior admissions experience
• ✔️ Long-term care experience preferred
