Unique Hospice and Palliative Care Is Hiring A Hospice Field RN

Hospice Field Registered Nurse (RN) – Mandarin / Cantonese Speaking Required 🎗️

Join our compassionate team at Unique Hospice as a dedicated Registered Nurse, providing end-of-life care with dignity, comfort, and respect in the field.

Responsibilities

• Provide direct patient care in accordance with the California Nurse Practice Act and hospice standards of care.
• Assess patient and family/caregiver needs and provide education on diagnosis, condition, medications, and care plans.
• Accurately and promptly document all visits, services, assessments, and condition changes, ensuring compliance with organizational and regulatory standards.
• Coordinate care with the interdisciplinary hospice team, reporting patient needs and concerns quickly.
• Participate actively in biweekly interdisciplinary team meetings.
• Conduct RN supervisory visits and recertification visits as required.
• Monitor patient conditions, communicating any changes that may influence the care plan.
• Maintain competency in nursing skills, hospice philosophy, symptom management, and patient assessment.
• Perform additional duties as assigned.

Qualifications

• Current California Registered Nurse license required.
• Must be fluent in Mandarin and/or Cantonese.
• Valid driver’s license, reliable transportation, and automobile insurance in accordance with California requirements.
• Strong understanding of hospice philosophy and the needs of terminally ill patients and their families.
• Excellent skills in communication, documentation, organization, and prioritization.
• Ability to work independently in the field while collaborating with the interdisciplinary team.
• Hospice experience is preferred but not mandatory.

• Salary: $110,000.00 - $120,000.00 per year
