🌟 Job Description

The Registered Nurse (RN) will deliver comprehensive nursing care aligned with each patient's care plan. Responsibilities include conducting health and psychosocial evaluations, monitoring patient conditions, promoting health and prevention, coordinating services, as well as teaching, training, and delivering direct nursing interventions.

📝 Position Responsibilities

  • Coordinate Patient Care: Perform thorough health and psychosocial evaluations, monitor patient status, promote preventive health practices, coordinate services, and lead educational activities.
  • Conduct Assessments: Complete initial and ongoing comprehensive assessments, including Outcome and Assessment Information Set (OASIS) evaluations at appropriate intervals.
  • Evaluate Effectiveness: Continuously assess the impact of nursing services on patients and families.
  • Perform OASIS Documentation: Handle admission, transfer, recertification, resumption of care, and discharge OASIS documentation.
  • Prepare & Present Records: Compile and present patient records to the Clinical Record Review Committee as needed.
  • Collaborate with Physicians: Consult with attending physicians regarding care plan modifications, coordinate with supervisors, and implement appropriate changes.
  • Participate in Case Conferences: Engage in discussions with supervisors about patient concerns and strategies.
  • Coordinate Interdisciplinary Care: Discuss additional team member involvement, such as therapists and social workers.
  • Documentation & Notes: Submit clinical notes within 48 hours, including progress and service documentation.
  • Develop & Revise Plans: Contribute to the creation and updating of physicians' POC and process change orders.
  • Support Staff: Assist or fill in for other nurses upon request by the Clinical Manager.
  • Supervise Aides: Supervise and evaluate Home Health Aides at least every 14 days, and provide written evaluations to the relevant departments.
  • Participate in Staff Development: Attend periodic staff meetings and coordinate with Aide supervisors regarding performance.

🎓 Qualifications

  • Working knowledge of home healthcare principles, nursing techniques, and necessary documentation.
  • Graduate of an accredited School of Nursing.
  • California RN license is required.
  • One or more years of experience in community/home health or hospital settings is preferred.
