This live-in position offers the opportunity to make a real difference by providing personalized in-home care to diverse clients, including seniors, individuals with disabilities, and those recovering from illness. 🏡💖

Job Overview

As a Home Health Care Aide, your dedication and caring nature will foster a warm and trusting environment. You will work closely with clients 6-7 days per week, including overnight shifts, ensuring they receive safe, respectful, and tailored support to promote independence and well-being.

Key Duties & Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and mobility support 🚿👗
• Monitor & document patients’ conditions: vital signs such as blood pressure, pulse, temperature, and respiration, accurately recording data in EHR/EMR systems 🩺📊
• Administer medication following prescribed guidelines, ensuring strict adherence to HIPAA privacy laws 💊🔒
• Support clients with specialized needs: Alzheimer’s, dementia, hospice, developmental disabilities, or other special care requirements 🧠❤️
• Operate medical equipment: including Hoyer lifts and perform catheter care as necessary 🏥🔧
• Prepare nutritious meals: and assist with meal planning to support health and wellness 🍽️🥗
• Perform light housekeeping: cleaning, laundry, and maintaining a safe environment at home 🧹🧺
• Maintain accurate medical records: detailed documentation for each client’s care plan 📝
• Collaborate with healthcare professionals: using EMR/EHR systems for seamless communication 🔄👩‍⚕️
• Provide emotional support and companionship: to enrich clients’ quality of life 💓😊
• Assist with hospice/end-of-life care: respecting each individual’s dignity and wishes ⚰️🤝

Qualifications & Requirements

• Proven experience in home health care or long-term care; experience in assisted living or nursing homes is highly valued 🏡👩‍⚕️
• Strong knowledge of medical terminology and procedures such as vital signs measurement, catheter care, medication administration, and CPR certification 🩺🔑
• Experience working with people with disabilities or special needs, including dementia or Alzheimer’s disease 👵🧓
• Familiarity with EMR/EHR systems for accurate documentation 📁
• Physical ability to safely lift patients using assistive devices, with stamina for long shifts 💪🛏️
• Excellent patient observation skills and ability to promptly identify changes 🧐
• Compassionate communication to build trust with clients from diverse backgrounds 💬✨
• Ability to work independently following detailed care plans; flexibility for live-in shifts or varied schedules 🕰️🔄
• Knowledge of HIPAA regulations to ensure confidentiality 🔒
• Valid driver’s license and reliable transportation are preferred for community visits 🚗✅
