Job Summary

This role offers the chance to work with a diverse range of populations, including seniors, individuals with disabilities, and those recovering from illness, in a warm and trusting environment.

🌟 Key Duties & Responsibilities

• Assist with Activities of Daily Living (ADLs) — including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient conditions — regularly recording vital signs (blood pressure, pulse, temperature, respiration) in EHR/EMR systems
• Administer medications — following prescribed guidelines while ensuring patient privacy and confidentiality in line with HIPAA standards
• Support clients with specialized needs — such as Alzheimer's, dementia, hospice, or developmental disabilities
• Operate medical equipment — including Hoyer lifts for safe transfers and perform catheter care when needed
• Meal preparation and planning — ensuring nutritious meals to promote health
• Light housekeeping tasks — like cleaning, laundry, and maintaining a safe living environment
• Accurate documentation — maintaining detailed medical records and care plans
• Collaborate with healthcare professionals — using EMR/EHR systems for seamless communication
• Provide emotional support and companionship — creating a positive and trusting environment
• Assist with hospice/end-of-life care — respecting the dignity and wishes of clients when necessary

📝 Qualifications

Currently, we are specifically seeking aides who speak Hindi or Punjabi.

• Proven experience in home health care or long-term care, including assisted living or nursing homes
• Strong knowledge of medical terminology and procedures like vital sign measurement, catheter care, medication admin, and CPR certification
• Experience working with individuals with disabilities or special needs, including dementia or Alzheimer's, is preferred
• Familiarity with EMR/EHR systems for documentation
• Physical ability to safely lift patients using Hoyer lifts or assistive devices — stamina for long shifts is essential
• Excellent patient observation skills to identify condition changes promptly
• Compassionate communication to build trust with clients from diverse backgrounds
• Ability to work independently following care plans; flexibility for live-in shifts or varied schedules
• Knowledge of HIPAA regulations to ensure confidentiality
• Valid driver’s license and reliable transportation are preferred for community visits
