We are seeking dedicated Home Care Supervisors to join our Long Term Care (LTC) team, making a meaningful difference in the lives of seniors and individuals with disabilities. We welcome bilingual candidates, with roles available for Vietnamese and Mandarin speakers.

Open Positions

  • Full-Time Vietnamese Speaking Home Care Supervisor — 37.5 hours/week, 8am - 4:30pm
  • Part-Time Mandarin Speaking Home Care Supervisor — 18-20 hours/week

Starting wages range from $33.06 - $36.78/hour (depending on experience), plus a $250 monthly stipend bonus!

How You Can Make a Difference 💙

The LTC Supervisor plays a vital role in ensuring the safety and well-being of frail seniors and individuals with disabilities residing at home. Reporting to the LTC Service Director, you will oversee a caseload of 80-100 clients and supervise 50-60 Home Care Assistants (HCAs). This position involves clinical advocacy, staff mentorship under a Union Collective Bargaining Agreement, and compliance with DOH regulations.

Responsibilities 📋

Client Intake & Advocacy

  • Conduct initial home visits to orient clients to their rights and policies
  • Perform safety inspections, develop personalized care plans, and emergency protocols

Personnel Management

  • Supervise and mentor 50-60 HCAs, including hiring, training, performance reviews, and corrective actions under union agreements

Case Coordination & Staffing

  • Manage caseloads, matching clients with trained HCAs
  • Monitor client satisfaction and ensure 24/7 coverage through rotation

Regulatory & Safety Compliance

  • Ensure adherence to DOH licensing and safety regulations
  • Serve as a mandated reporter and community liaison

Administrative Oversight

  • Manage client and HCA data, verify electronic visit records for payroll and billing
  • Maintain organized digital and paper files
  • Protect PHI/ePHI: Lead privacy initiatives, monitor staff access to sensitive information, and educate staff on privacy responsibilities

Qualifications 📜

Minimum Qualifications

  • Bachelor's Degree in Business, Healthcare, or related field, with 4+ years of experience in fast-paced healthcare delivery (experience may substitute education)
  • Home Care Aide License, Nursing Aide Certification, or equivalent
  • Excellent communication and interpersonal skills
  • Strong organizational and time management skills
  • Ability to work independently and as part of a team
  • Experience in health services and working with diverse populations
  • Valid Driver's License and auto insurance
  • Pass criminal background check

Preferred Qualifications

  • 3+ years supervising Home Care, Home Health, or Hospice programs
  • Proficiency with computer programs
  • Experience with Medicaid home care services
  • Familiarity with the service community
