What You'll Do as a Patient Care Coordinator 📝

• Serve as the main point of contact: for patients and families
• Coordinate personalized care plans: based on individual needs
• Manage scheduling, follow-ups, and assessments in-home
• Educate patients and caregivers: on routines and available services
• Track patient progress: and maintain accurate documentation

What We're Looking For 🔍

• At least 1+ year of experience in care coordination, case management, or clinical support (home care preferred)
• Mandarin-speaking required; Cantonese and Fuzhounese-speaking desired
• Strong communication and organizational skills
• Proficiency in Microsoft Office and EHR systems
• Ability to multitask in a fast-paced environment
