HEALTH HOME PLUS CARE MANAGER

Join our team as a Health Home Plus Care Manager and play a vital role in coordinating healthcare services and supporting members' well-being across NYC.

Responsibilities

• Enrollment Activities: Perform all enrollment tasks as mandated by the Department of Health (DOH) and Lead Health Home requirements, including outreach, obtaining consents, and gathering supporting documentation.
• Assessments & Care Planning: Conduct comprehensive assessments and be responsible for creating and managing the member's Plan of Care (POC).
• Care Coordination: Coordinate care among providers and community resources. Assist with service coordination during hospitalizations or facility admissions. Facilitate discharges and transfers to ensure continuity of care.
• Team Collaboration: Lead Interdisciplinary Team Meetings to develop and update the Plan of Care and coordinate services.
• Public Benefits Assistance: Support members in obtaining and maintaining benefits such as Medicaid, Social Security, SNAP, housing, legal, and employment services.
• Member Education: Educate members, caregivers, and support persons on diagnoses, conditions, needs, strengths, and goals.
• Advocacy: Act as an advocate for members within institutional settings, in the community, or when interacting with providers and resources.
• Referral & Follow-up: Make referrals to services, ensuring members follow through with appointments and services.
• Documentation & Billing: Meet monthly billing requirements, perform home and field visits, and maintain accurate case records of all services provided.
• Supervision & Additional Duties: Attend regular supervision sessions and perform any additional duties as assigned.

Requirements

• Must be available to work 40 hours per week in a full-time capacity.
• Willing to travel within select NYC boroughs: Bronx, Manhattan, Brooklyn, Queens, Staten Island, and Westchester.
• Educational & Experience Qualifications:
  • Master’s degree in a qualifying field + 1 year of experience, OR
  • Bachelor’s degree in a qualifying field + 2 years of experience, OR
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) + 2 years of experience, OR
  • Bachelor’s degree or higher in any field + 3 years of experience, OR
  • Two years of experience as a Health Home care manager serving the SMI or SED population.
• Proficient in modern communication tools including computers, Office 365, and Healthcare Management systems.
• Preferred bilingual candidates with proficiency in English and additional languages such as Spanish, French, Haitian Creole, Arabic, Cantonese, Mandarin, or Bengali.

Compensation & Benefits

Starting Pay: $30.00 per hour

Additional Details
