Position Summary

The Care Coordinator in Penn Signature Services (PSS) plays a crucial role in delivering exceptional service to prospective and current patients accessing UPHS/Penn Medicine's remote and in-person services. 🌍 They act as the primary point of contact and liaison, seamlessly coordinating service and medical care while managing communications with referring parties such as university trustees, senior leaders, government agencies, insurance providers, and corporate partners.

Key Responsibilities

  • Patient Liaison & Coordination: Serve as the primary contact, understanding patients' needs, coordinating with departments, and facilitating communication among stakeholders. Handle high-profile, destination, or premium patients requiring complex scheduling and coordination, including telehealth options and interpreter services as needed.
  • Communication & Documentation: Communicate with patients regarding financial considerations, ensuring privacy and HIPAA compliance. Maintain detailed records of activities, updates, and referrals in PSS databases like Salesforce. Prepare and share reports on patient activity and service utilization.
  • Pre- and Post-Visit Engagement: Meet patients prior to appointments, assist with scheduling, and facilitate navigation during visits, including outside office hours if necessary. Facilitate clinical requests such as transfers, equipment, pharmacy needs, and transportation arrangements.
  • Accommodation & Financial Support: Coordinate hotel accommodations and assist with initial financial account setup for international patients.
  • Quality & Service Improvement: Monitor patient satisfaction, identify issues, implement corrective actions, and participate in quality initiatives and training sessions. Manage patient relation issues non-clinically, forwarding clinical inquiries to appropriate staff.
  • Compliance & Policies: Perform all duties in line with Penn Medicine values and policies, ensuring HIPAA and privacy standards are upheld.

Qualifications & Experience

  • Required: H.S. Diploma/GED, 3+ years in customer service or client interaction
  • Preferred: Bachelor’s degree, bilingual capabilities
