Responsibilities 📝
- Partner with families to conduct assessments, set goals, provide interventions, and manage cases to prevent foster care placement and promote well-being.
- Learn and adhere to the Family Treatment & Rehabilitation (FTR) model through training, coaching, and case consultations.
- Maintain fidelity to the FTR practice model across case contacts, interventions, supervision, documentation, and treatment duration.
- Collaborate closely with an interdisciplinary team including Mental Health and Substance Abuse Specialists, Case Aides, and FTC Facilitators.
- Participate in Clinical Diagnostic Team Meetings to develop effective treatment plans.
- Offer intensive home-based services and accompany clients to appointments as needed.
- Provide concrete support such as advocacy, outreach, and referrals.
- Assess safety and risk levels, and develop crisis management and safety plans accordingly.
- Perform toxicology tests as necessary.
- Conduct family conferences using the Family Team Conferencing (FTC) model, coordinating with community partners and ACS.
- Collaborate with ACS, schools, and collateral providers to support client needs.
- Plan, coordinate, and lead social activities and groups for clients.
- Complete all documentation, including assessments, progress notes, treatment plans, and closing summaries, within established deadlines, utilizing mobile technology.
- Participate in supervision, meetings, and training sessions.
- Demonstrate cultural sensitivity and contribute to creating a respectful learning environment.
- Perform other duties as assigned.
Qualifications 🎓
- BA or BSW with experience; MSW or MA in a related field with a willingness to obtain licensure is preferred.
- Fluency in Spanish is required; bilingual candidates are strongly encouraged to apply.
- Some evening hours may be required.
- Excellent organizational, written, and verbal communication skills.
- Proficiency in Microsoft Office Suite.
- Commitment to a strength-based approach in service delivery.
Compensation 💰
- Standard Salary: $57,000 / $31.32 per hour
- Bilingual Salary: $59,000 / $32.42 per hour
