Preventive Case Planner – Family Treatment & Rehabilitation Program

Overview

Serving Queens, the Case Planner provides supportive counseling and case management to enhance safety, reduce risks, and promote wellbeing. This role utilizes the Family Treatment & Rehabilitation (FTR) model of practice, working closely with families impacted by mental illness and/or substance abuse.

The Role

The Case Planner collaborates with the Supervisor and Mental Health & Substance Abuse Specialists to ensure all responsibilities adhere to model fidelity and standards set by FTR, ACS, and SCO Family of Services. We offer strong supervision and rich professional development opportunities.

Responsibilities 📝

  • Partner with families to conduct assessments, set goals, plan interventions, and manage cases to prevent foster care and promote wellbeing.
  • Learn the FTR practice model by participating in training, coaching, and case consultations.
  • Maintain fidelity to the FTR model: Follow structured case contacts, interventions, supervision, documentation, and treatment length guidelines.
  • Collaborate with an interdisciplinary team including Mental Health & Substance Abuse Specialists, Case Aides, and FTC Facilitators.
  • Participate in Clinical Diagnostic Team Meetings to develop treatment plans.
  • Provide intensive home-based services and accompany clients to appointments as needed.
  • Deliver concrete services such as advocacy, outreach, and referrals.
  • Assess safety and risk, implement crisis and safety plans.
  • Conduct toxicology testing as necessary.
  • Coordinate conferences with families, community partners, and ACS using the Family Team Conferencing (FTC) model.
  • Collaborate with ACS, schools, and other collateral providers.
  • Plan and facilitate group and social activities for clients.
  • Complete all documentation and data entry accurately and timely, including assessments, progress notes, treatment plans, and closing summaries.
  • Utilize mobile technology to complete required documentation.
  • Attend supervision, meetings, and trainings to support ongoing professional growth.
  • Work collaboratively with Case Aides and FTC Facilitators.
  • Demonstrate cultural sensitivity toward diverse backgrounds and socioeconomic statuses.
  • Contribute positively to the program's vision and mission.
  • Create and support a respectful, learning-oriented environment.
  • Perform other duties as assigned.

Qualifications ✅

  • BA or BSW with experience; MSW or MA in related field with commitment to obtaining licensure preferred.
  • Seeking bilingual candidates fluent in Spanish.
  • Availability for some evening hours.
  • Excellent organizational, written, and verbal communication skills.
  • Proficient in Microsoft Office suite.
  • Strong commitment to a strength-based perspective.

Compensation 💰

Standard Salary
$57,000 / $31.32 per hour
Bilingual Candidates
$59,000 / $32.42 per hour
