Come join our team and make a positive impact on the lives of others! We’re seeking a Registered Nurse - Bilingual Spanish for our Licensed Home Care Bronx team.

Department & Position Overview

The Field Registered Nurse is a vital member of the FreedomCare Licensed Home Care team. Responsibilities include assessing patients’ current service needs and reevaluating care plans whenever significant health changes occur.

Every Day You Will

• Conduct initial assessments to determine care needs and develop personalized care plans.
• Train and supervise non-skilled caregivers in personal care, companionship, and daily living activities.
• Monitor clients’ overall well-being and report changes to healthcare professionals.
• Educate clients and families on effective care strategies, safety, and daily living skills.
• Ensure compliance with agency policies, state regulations, and best practices in non-skilled care.
• Maintain accurate documentation of client care and progress, safeguarding confidentiality.
• Act as a liaison between clients, families, and caregivers to facilitate communication and improve care quality.
• Participate in ongoing training and professional development to stay current with best practices in home care.

Ideal Candidate Will Possess

• Bachelor's Degree (BSN) and a current NYS license or compact license in good standing OR associate's degree in nursing with relevant homecare clinical experience.
• 1-2 years of nursing experience, particularly in homecare settings.
• Experience with geriatric care or individuals with disabilities.
• Ability to travel within the 5 boroughs as needed.
• Knowledge of state and federal home health regulations.
• Strong interpersonal and communication skills.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook, etc.).
• Excellent verbal and written communication, organizational, and time management skills.
• Highly Preferred: Bilingual in Spanish, Russian, Chinese, Korean, or Bengali.
• Ability to work collaboratively with caregivers and families.
• Proficiency with electronic health record (EHR) systems.
• Valid driver’s license and reliable transportation.
• Experience in non-skilled home care and with the Uniform Assessment System (UAS).

Nice-to-Haves

• Master's degree in nursing.
• Experience in home care, geriatrics, and case management with field assessments.
• 3+ years of agency experience in home care.
