Salary Range: $50,000 - $55,000

📝 Job Summary
The Family Partner works directly with families receiving clinical services, leveraging lived experience as a caregiver for children with special or mental health needs. They mentor families, help navigate child-serving systems, and support service access and care planning.
🎯 Duties and Responsibilities
- Provide education and support during treatment; attend care coordination meetings; assist caregivers in articulating youth's strengths, needs, and goals.
- Educate caregivers on navigating child-serving systems and accessing community resources, including informal supports.
- Support parents/caregivers in navigating systems, fostering empowerment, and linking to peer/support groups.
- Coordinate care through phone, in-person, and virtual meetings; facilitate appropriate referrals and "warm hand-offs" to providers.
- Identify community resources and develop natural supports to help youth achieve treatment goals.
- Collaborate within a multidisciplinary team, including clinical leaders and counselors, to support and empower families.
🎓 Minimum Qualifications
- High school diploma or HISET, with at least two (2) years’ experience working with children, adolescents, or transitioning youth.
- Experience as a caregiver for youth with special needs, preferably with mental health challenges.
- Familiarity with family-centered, strength-based services, and navigating child and family-serving systems.
- Innovative, creative, and decisive problem-solving skills.
- Strong communication, organizational, and administrative capabilities.
- Ability to work effectively with diverse populations and community agencies (e.g., School District, DYS, Juvenile Court).
- Understanding and willingness to support youth and families 24/7, 365 days a year.
- Valid driver’s license and vehicle availability.
- Bilingual in Spanish/English or Portuguese.
