Overview

The Coordinated Family Support (CFS) Specialist will identify and provide essential supports to help adults with developmental disabilities live successfully in their family homes. This role involves working closely with individuals and their families to develop personalized action plans ensuring success in the community. CFS services encompass connecting clients with community resources, offering assistance and navigation support, and providing futures planning and training.

The focus is on enhancing equitable access to services and reducing ethnic and racial disparities by tailoring services to respect the cultural, ethnic, and linguistic preferences of the individuals and families served.

Key Responsibilities

  • Identify and provide supports necessary for successful family home residency.
  • Assist and train consumers and families to navigate comprehensive services tailored to their needs, including overcoming access barriers.
  • Provide information and resources related to the consumer’s diagnosis and identified needs.
  • Ensure consistency in training across providers based on consumer and family needs.
  • Coordinate service scheduling, including medical and other appointments.
  • Identify transportation options and assist with arrangements.
  • Identify backup providers and supports to ensure continuous service.
  • Engage in futures planning for consumers, especially those living with aging caregivers.
  • Train consumers and community stakeholders to promote independence.
  • Prepare and submit quarterly reports on service progress.
  • Perform general administrative and support duties as assigned.

Qualifications

  • Associate’s degree in a human services field OR at least 3 years of experience in developmental disabilities service delivery.
  • Ability to communicate effectively in the primary language of the consumer and their family.
  • Criminal Record clearance.
  • First Aid/CPR certification.

Compensation & Benefits 💰

Pay: $20.00 - $25.00 per hour

  • Education: Associate degree (preferred)
  • Experience: Working with disabled individuals, 3 years (preferred)
  • Language: Dari or Farsi (preferred)
