Responsibilities 📋

The Community Liaison plays a pivotal role in inbound and outbound communication, business development, and community education. This role involves building relationships with community partners, educating patients and caregivers about our services, and facilitating patient transitions of care.

Key Duties include:

  • Performing market analysis and understanding the competitive landscape to differentiate HarmonyCares.
  • Educating the community about our mission to provide high-quality, personalized healthcare in the home.
  • Achieving and exceeding scheduled in-person and virtual meetings with decision makers.
  • Understanding eligibility and enrollment processes thoroughly.
  • Assisting with benefit enrollment and related processes as needed.
  • Tracking performance metrics in CRM systems to meet regional targets.
  • Scheduling and conducting discussions with patients and caregivers regarding program eligibility.
  • Informing and assisting patients interested in accountable care organizations or CMS alternative payment models, including collecting voluntary alignment forms.
  • Handling inbound calls from patients, families, and facility staff, addressing inquiries related to care, scheduling, and billing.
  • Documenting outreach efforts, scheduling meetings with external sources, and maintaining records.
  • Adhering to company policies, marketing guidelines, and all legal requirements, including HIPAA privacy standards.
  • Maintaining active communication with sales leaders and support staff.
  • Performing additional duties as assigned.

Qualifications & Skills ✅

  • Bachelor’s Degree or 4+ years of B2B sales experience
  • At least 2 years of healthcare industry sales or marketing experience
  • Valid driver’s license and a good driving record
  • Strong organization, problem-solving, and territory management skills
  • Excellent interpersonal, presentation, and customer service abilities
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
  • Willingness to travel within the assigned territory and attend meetings
  • Exceptional written and verbal communication skills
  • Ability to work independently, multitask, and stay organized
  • Self-motivated and driven, requiring minimal oversight

Compensation 💰

Salary Range: USD $65,000.00 to $100,000.00 annually

Note: Individual compensation packages are determined based on candidate’s skills, experience, and qualifications, respecting various factors and considerations.
