Responsibilities
The Community Liaison plays a vital role in communication, business development, and patient engagement. You will serve as the local market representative, educating community partners and potential patients about our services and mission, facilitating patient care transitions, and increasing enrollment. 🚑
Key Duties Include:
- Conducting market analysis and understanding the competitive landscape to differentiate HarmonyCares services
- Educating the community about our mission and personalized healthcare in the home
- Meeting or exceeding goals for in-person and virtual meetings with decision-makers
- Understanding eligibility and enrollment processes thoroughly
- Facilitating benefit enrollment and guiding patients through processes
- Tracking performance metrics in CRM systems within regional targets
- Scheduling discussions with patients and caregivers to assess eligibility
- Informing interested patients about accountable care organization programs and collecting voluntary forms for alignment
- Handling inbound calls from patients, families, and facility staff regarding services, scheduling, and billing
- Triage of patient concerns related to care needs swiftly and professionally
- Documenting outreach activities and meetings with external sources
- Adhering to company policies, legal requirements, and HIPAA guidelines
- Maintaining communication with sales leaders and support staff
- Performing additional duties as assigned
Qualifications
Required Knowledge, Skills, and Experience:
- Bachelor’s Degree or 4+ years of B2B sales experience
- Minimum of 2 years in healthcare-related sales or marketing
- Valid driver’s license and good driving record
- Strong organization and problem-solving skills
- Excellent interpersonal, presentation, and customer service abilities
- Proficiency with Microsoft Office Suite (Word, Excel, Outlook)
- Willingness and ability to travel within the assigned territory
- Outstanding written and verbal communication skills
- Capable of working independently and managing multiple tasks
- Highly organized and self-motivated
Compensation
Pay Range: USD $65,000.00 to $100,000.00 per year 💰
