Position: Community Liaison 🤝
The Community Liaison manages inbound and outbound communication and supports business development efforts. This role acts as the local market representative, educating community partners and potential patients about HarmonyCares, our mission, and services. It also facilitates patient care transitions and explains enrollment benefits to eligible individuals and caregivers.
Essential Duties & Responsibilities 🎯
- Conduct market analysis and evaluate the competitive landscape to differentiate HarmonyCares services to referral partners
- Educate the community about our mission to provide high-quality, personalized healthcare in the home
- Achieve or exceed meeting goals with key decision makers through in-person and virtual engagements
- Understand and explain eligibility and enrollment processes
- Facilitate benefit enrollment and provide educational support as needed
- Track performance metrics within CRM systems to meet regional targets
- Coordinate discussions with patients and caregivers regarding program eligibility
- Inform interested patients about accountable care organizations and assist with voluntary alignment documentation
- Respond promptly to inbound inquiries from patients, families, and facility staff
- Triages patient concerns related to care, scheduling, and billing to appropriate departments
- Document outreach efforts and scheduled meetings with external partners
- Adhere to all company policies, legal requirements, and healthcare regulations, including HIPAA
- Maintain consistent communication with sales leaders and support staff
- Perform additional duties as assigned
Qualifications & Skills ✅
- Bachelor’s Degree or 4+ years of B2B sales experience
- 2+ years in healthcare-related industry sales or marketing
- Valid driver’s license with a good driving record
- Strong organizational and problem-solving skills
- Excellent interpersonal, presentation, and customer service skills
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
- Willingness to travel within the assigned territory and attend sales meetings
- Excellent written and verbal communication abilities
- Ability to work independently and manage multiple tasks effectively
- Highly motivated and self-driven, requiring minimal oversight post-training
Compensation & Benefits 💰
Minimum Annual Salary: $65,000
Maximum Annual Salary: $100,000
Note: Compensation is determined based on individual skills, experience, and qualifications, among other factors.
