Key Responsibilities 🎯
The Community Liaison plays a vital role in inbound and outbound communication, business development, and community engagement. Your mission will include educating community partners and potential patients about our services, facilitating patient care transitions, and promoting enrollment benefits.
Essential Duties & Responsibilities
- Conduct market analysis and understand the competitive landscape to differentiate HarmonyCares services to referral partners
- Educate the community on our mission to deliver high-quality, personalized healthcare in the home
- Achieve or surpass in-person and virtual meeting goals with key decision-makers
- Understand and explain eligibility and enrollment processes
- Provide education on benefit enrollment and facilitate appropriate processes
- Track performance using CRM tools, ensuring goals and budgets are met within your region
- Schedule and conduct discussions with patients and caregivers regarding program eligibility
- Inform patients about participation in CMS programs like ACOs; collect voluntary alignment forms if they choose to participate
- Handle inbound calls from patients, families, and facility staff with professionalism and care
- Address patient concerns related to care, scheduling, and billing, directing them to the appropriate departments
- Document outreach activities and meetings with external partners
- Maintain compliance with all company policies, laws, and regulations, including HIPAA privacy and security standards
- Communicate regularly with sales and leadership teams, providing updates and support
- Perform additional duties as assigned
Qualifications & Skills ✨
- Bachelor’s Degree or 4+ years of B2B sales experience
- Minimum of 2 years in healthcare-related sales or marketing
- Valid driver’s license with a good driving record
- Strong organizational, problem-solving, and territory management skills
- Excellent interpersonal, communication, and presentation skills
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
- Willingness to travel within assigned territory and attend sales meetings
- Self-motivated, independent worker capable of managing multiple tasks
- Driven individual with minimal need for oversight after initial training
Compensation 💰
Annual salary ranges from USD $65,000 to USD $100,000, based on experience and qualifications.
