Community Health Worker - Full Time Opportunity

Join a dedicated team and make a difference in your community as a Community Health Worker. This full-time position offers a rewarding day shift from 8:00 AM to 4:30 PM.

Job Summary

Provides direct services through education, advocacy, and service coordination to individuals, supporting their psychosocial well-being and connecting them with vital resources.

Essential Functions

• Assessment and Planning:

  Gather information about clients' personal, financial, health, and emotional status to develop tailored plans addressing psychosocial barriers. Create client-directed Service Plans outlining needed services.

• Education and Support:

  Offer education and psychosocial support on topics such as health, medical compliance, budgeting, safety, parenting, and community resources based on assessed needs.

• Advocacy and Services Coordination:

  Maintain up-to-date information on available community resources, facilitate timely referrals, and coordinate ongoing services to meet clients' needs.

• Documentation:

  Accurately maintain all documentation required by hospital policies, departmental standards, and funding agencies.

• Outreach:

  Collaborate with staff to provide information and referrals to the community and individuals in both community settings and in-home environments.

Requirements

Formal working knowledge comparable to an Associate's degree (2 years of college), with expertise in a specialized field.
