We are seeking a passionate Community Health Worker (CHW)/Care Gap Specialist to join our healthcare team and make a meaningful impact in the community.
Position: Community Health Worker (CHW)/Care Gap Specialist in a Healthcare Setting
🔍 Position Summary
The CHW/Care Gap Specialist acts as a trusted liaison between patients, the healthcare care team, and community resources. Embedded within a healthcare setting, this role involves supporting Medicaid enrollment and renewals, addressing social determinants of health (SDOH), and closing gaps in preventive and chronic care. This referral-driven role focuses on targeted patient engagement to improve health outcomes.
🌟 Key Responsibilities
📝 Medicaid Enrollment & Renewal
- Assist patients with Medicaid applications and renewals via COMPASS and Pennie
- Educate patients on eligibility, benefits, and coverage requirements
🤝 Referral-Based Patient Engagement
- Conduct outreach (in-person, phone, or virtual) to assess patient needs
- Document activities and outcomes in the EHR or designated system
🩺 Social Determinants of Health (SDOH)
- Conduct screenings and connect patients to resources for housing, food, transportation, utilities, and employment
- Follow up to confirm service access and barrier resolution
⚕️ Care Gap Closure
- Identify patients overdue for screenings, immunizations, and chronic care follow-ups
- Assist with appointment scheduling and transportation coordination

- Proficiency in MS Office, EHR systems, and web-based tools
- Flexibility to work evenings and weekends as needed

🔎 Additional Requirements
- Bilingual (English/Spanish) required
🎓 Education & Experience
- High School Diploma required; Associate or Bachelor’s degree preferred
- Experience in Medicaid, community outreach, health education, or social services preferred
📜 Licensure & Certification
- CHW certification or ability to complete training within 6 months of hire
- Obtain Pennsylvania CHW certification within 1 year of training start
- Pennie Enrollment Assister certification required post-hire
- CPR certification required
💪 Physical Requirements
- Ability to travel within the community and attend meetings
- Prolonged periods of sitting and computer use
- Frequent communication with patients, care teams, and community partners
- Ability to observe details and maintain accurate documentation
