Who You Are:
- You reside in the United States and are fluent in Russian, Vietnamese, or Chinese.
- You hold valid credentialing as a Certified/Registered Medical Assistant, LPN/LVN, or similar clinical qualifications.
- You are dependable, motivated, and enjoy working with the Medicare population. 🩺
- Familiarity with Chronic Care Management and/or Remote Patient Monitoring is a plus.
- You work efficiently independently in a HIPAA-compliant home environment and deliver empathetic, patient-focused care.
- Your customer service skills are excellent—you build rapport through meaningful, relevant phone interactions.
- You are comfortable using and troubleshooting technology, including setup and access to provided equipment. (No programming required!)
- You are conscientious with excellent time management and committed to the values of integrity and excellence.

How You'll Contribute:
- Independently managing a monthly caseload, ensuring each patient receives quality care.
- Optimizing HealthXL's CCM/RPM platform for service consistency and compliance.
- Navigating multiple systems (CCM/RPM platform, EMRs, 8x8, etc.) and accurately documenting each interaction.
- Making multiple outreach attempts to contact patients, aiming to speak with at least 90%, and updating their Care Plans with the latest information for their doctors. 📋
- Collaborating with a team of compassionate clinicians dedicated to providing excellent non-face-to-face patient care from home.

Clinical Care Coordinator REMOTE - CCM/RPM - Multilingual (Russian, Vietnamese, or Chinese) - Overview
At HealthXL, we deliver Virtual Care Services within the framework of Medicare's CCM and RPM programs. This role is suitable for Medical Assistants, RNs, LPNs, or other qualified clinicians adept at providing telephonic, non-face-to-face care as an extension of the physician practice. 📱🏥
The role involves navigating various EMR platforms, creating comprehensive, patient-centered care plans, conducting monthly health reviews, coordinating with family members, caregivers, and specialists. This flexible, remote position can be part-time (20 hrs/wk) or full-time (40 hrs/wk), with virtual paid training for four days. All necessary equipment, including computers, monitors, and headsets, is provided. The position reports to the Manager, Service Quality & Compliance.
Responsibilities
- Meet and exceed production goals outlined in the Performance Standards.
- Maintain secure, HIPAA-compliant access to systems, handling PHI with utmost confidentiality.
- Follow iCare standard workflows and questionnaires to maximize patient interactions and data accuracy.
- Conduct monthly telephonic interactions with patients, ensuring regular engagement and trust-building.
- Provide compassionate, consistent care that fosters trust and rapport.
- Communicate relevant information effectively with the practice on behalf of patients.
Qualifications
- Alignment with iCare's core values.
- Residency in the United States.
- Clinical credentials such as MA, LPN, RN, BSN, or other approved backgrounds.
- Multi-state compact nursing license (if applicable).
- Fluency in Russian/Ukrainian, Chinese, or Vietnamese.
- At least 1 year of experience in a clinical or patient-facing environment.
- Excellent verbal communication skills with empathy and active listening.
- Strong understanding of medical terminology.
- Comfort in remote patient assessment and recognition of warning signs.
- Organized with thorough documentation skills.
