How You'll Contribute 🚀

• Manage a caseload of patients independently, ensuring timely and effective care for each individual 📋
• Optimize the use of our CCM/RPM platform to deliver consistent, compliant services 🖥️
• Navigate multiple systems, including CCM/RPM platform, EMRs, and communication tools like 8x8 📲
• Document all interactions accurately and comprehensively each month 📝
• Connect with at least 90% of your assigned patients via phone, making multiple contact attempts and updating their Care Plans 📞

Position Overview: Clinical Care Coordinator – CCM/RPM (Remote) 🌍

At HealthXL, we provide virtual clinical services under Medicare’s CCM and RPM programs. This role extends the physician’s care through telephonic, non-face-to-face interactions, requiring clinical credentials such as MA, LPN, RN, or BSN. You will navigate multiple electronic health record platforms, create comprehensive care plans, and coordinate follow-up care, including health reviews, specialist referrals, and lab coordination.

This flexible position can be part-time (20 hours/week) or full-time (40 hours/week). All necessary equipment, including computers, monitors, and headsets, is provided, along with a virtual paid training program scheduled over four days. The role operates under the supervision of the Manager of Service Quality & Compliance.

Responsibilities 🎯

• Meet and exceed production goals outlined in performance standards
• Maintain HIPAA-compliant and secure handling of Protected Health Information (PHI) 🛡️
• Follow established workflows and questionnaires to maximize patient engagement and documentation 📑
• Conduct monthly telephonic interactions with patients, focusing on building trust and providing empathetic care ❤️
• Communicate effectively with patients and practice teams to ensure seamless care coordination 📞

Qualifications ⭐

• Reside within the United States 🇺🇸
• Clinical credentials (e.g., MA, LPN, RN, BSN) and multi-state nursing license if applicable 🎓
• Fluent in Russian, Ukrainian, Chinese, or Vietnamese 🗣️
• Minimum 1 year of experience in a clinical or patient-facing environment 🏥
• Strong verbal communication skills and empathy 💬❤️
