How You'll Contribute: 🚀

  • Manage a monthly caseload of patients, ensuring timely and effective care for each individual.
  • Optimize and navigate the HealthXL CCM/RPM platform for consistency and compliance in service delivery.
  • Become proficient in multiple screens, including the CCM/RPM platform, EMRs, and communication tools like 8x8.
  • Document all interactions meticulously, ensuring accurate and detailed records each month.
  • Aim to contact at least 90% of your assigned patients, making multiple outreach attempts per patient.
  • Update and create individual Care Plans to keep physicians informed and improve patient outcomes.

Position Overview: Clinical Care Coordinator (Remote - CCM/RPM) 🌐

HealthXL provides virtual care services under Medicare’s CCM and RPM programs, supporting patients across the U.S. as an extension of their healthcare providers. This role requires clinical credentials such as Medical Assistant, RN, LPN, or similar qualifications.

The ideal candidate is skilled in providing telephonic, non-face-to-face care, navigating multiple EMR platforms, and creating comprehensive, patient-centered care plans. Responsibilities include conducting monthly health reviews, coordinating with caregivers and specialists, and maintaining detailed documentation.

This flexible position is available part-time (20 hours/week) or full-time (40 hours/week). Paid virtual training lasts four days, and all necessary equipment is provided. The role reports to the Manager of Service Quality & Compliance.

Responsibilities: 📋

  • Meet or exceed productivity goals outlined in performance standards.
  • Maintain HIPAA-secure data handling and communication practices.
  • Follow standard workflows and questionnaires to maximize patient engagement and documentation accuracy.
  • Perform monthly telephonic interactions with patients, fostering trusting relationships through compassionate communication.
  • Communicate relevant health information effectively to the practice on behalf of patients.

Qualifications: ✅

  • Demonstrates HealthXL’s core values
  • Resides in the United States
  • Holds clinical credentials such as MA, LPN, RN, BSN, or others approved
  • Possesses multi-state nursing license if applicable (LPN, RN, BSN)
  • Fluent in Russian/Ukrainian, Chinese, or Vietnamese
  • Minimum of 1 year experience in a clinical or patient-facing environment
  • Excellent verbal communication skills and empathy
  • Strong understanding of medical terminology
  • Comfortable remotely assessing and interacting with patients, with active listening skills
  • Able to recognize warning signs of potential health issues
  • Detail-oriented with thorough documentation skills
