As a Client Care Coordinator, you will play a vital role in supporting individuals and families in need, showcasing your communication, organizational, and problem-solving skills while making a meaningful difference.

Responsibilities 📝

  • Collaborate with patients, referral sources, caregivers, and MLTC to coordinate home placement, address issues, and identify necessary home health services.
  • Schedule and coordinate home care services for clients.
  • Maintain communication with MLTC / insurance providers regarding patient status and updates.
  • Keep accurate client records and documentation, ensuring confidentiality and compliance.
  • Conduct monthly service calls to caregivers and patients, updating MLTC about hospitalizations or vacations.
  • Utilize strong analytical skills to manage multiple tasks concurrently.
  • Assist with timesheet verification and related processes.
  • Staff cases when caregivers are unavailable or call out.
  • Serve as the main point of contact for clients, families, and external stakeholders, providing guidance, support, and information.
  • Provide rotating on-call services during non-working hours, weekends, and holidays.
  • Perform additional duties as needed to support agency operations.

Required Skills & Abilities 🚀

  • Ability to multitask effectively.
  • Flexibility to rotate on-call duties weekly and on weekends.
  • Proficiency in MS Excel and Word.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and customer service abilities.
  • Exceptional organizational skills with keen attention to detail.
  • Effective time management skills with a proven track record of meeting deadlines.
  • Strong analytical and problem-solving capabilities.

Physical Requirements 💪

  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to carry, lift, push, or pull at least 20 lbs.

Education & Experience 🎓

  • At least one year of experience in Home Care or a healthcare setting preferred.
  • Bachelor's degree preferred.
