Position: School Age Service Coordinator

Summary: The School Age Service Coordinator will provide case management and support families through educational and support planning. The primary focus is promoting family unity and maximizing educational opportunities for children of school age.

Essential Job Functions

  • Provide case management and referrals across social service areas such as early childhood education, after-school programs, youth and young adult development, financial education, workforce development, community building, and health & wellness.
  • Schedule and facilitate IPP meetings, annual and semi-annual reviews, transition planning conferences, clinical consultations, and ID/Team meetings.
  • Offer resources for age-appropriate activities and community events.
  • Maintain regular contact with children served and develop individual program plans (IPP).
  • Provide information and referrals to community services and resources.
  • Engage individuals and families in person-centered planning and support activities.
  • Support families with advocacy, education, crisis intervention, and other support services.
  • Serve as a liaison between service providers, care homes, schools, and community agencies.
  • Coordinate the Person-Centered Planning process with families.
  • Develop and oversee Individual Program Plans (IPP).
  • Counsel individuals on setting and achieving personal goals.
  • Advocate for individuals and families to ensure necessary services are provided.
  • Follow up on referrals to service providers and identify unmet needs.
  • Perform tasks related to fiscal accountability and caseload documentation.
  • Access and share resources for individuals across various counties.
  • Provide crisis support to promote stability.
  • Conduct visits to residential facilities or day programs as needed, including annual and quarterly assessments.
  • Evaluate conservatorship requests and prepare reports.
  • Perform home visits and community meetings in various settings.

Requirements

Education & Experience

• Bachelor’s Degree in Social Work, Rehabilitation Counseling, or a related field from an accredited college.

• Or a Bachelor’s Degree in Social Work plus four years of relevant experience.

Note: The Executive Director may waive specific educational or experience requirements to meet agency needs, such as serving specific cultural or language groups.

Skills & Knowledge

  • Understanding of social work principles, human development, and family dynamics.
  • Knowledge of intervention techniques, community resources, and organizational skills.
  • Ability to relate constructively with clients, families, colleagues, and community partners.
  • Strong communication skills for reports and presentations.
  • Capable of maintaining clear, concise case records.

Salary & Compensation

Monthly Salary Range: $6,101 – $7,422 (dependent on experience)
