- Salary Range: $53,000 - $65,000 annually 💰
Program Overview
The Community Supportive Housing Program provides permanent supportive housing to individuals and families transitioning from homelessness. Using the Housing First model, we aim to lower barriers to housing and offer comprehensive support to highly vulnerable, chronically homeless residents. As a Case Manager, you will coordinate care, develop life skills, support tenancy, and facilitate referrals to behavioral health services.
Key Responsibilities
- Trauma-Informed Support: Collaborate with the Program Director and multidisciplinary team to provide individualized, trauma-informed assistance that helps residents maintain housing.
- Client Engagement: Build rapport, assess ongoing needs, and make service referrals to promote housing stability.
- Service Provision: Offer diverse client services, including resource referrals, life skills training, health maintenance, medication monitoring, and transportation to appointments 🚍.
- Documentation: Keep client charts and files updated, documenting all communications in the Homeless Management Information System (HMIS).
- Housing Stability: Coordinate with landlords and apartment managers, negotiate leases, and complete annual certification documents for the County.
- Home Inspections: Conduct monthly home inspections to ensure safety and compliance.
- Coordination & Record Keeping: Maintain communication with Montgomery County, landlords, and building management.
- Meetings & Community Engagement: Attend county meetings, including monthly team and housing prioritization discussions, along with staff and Interfaith Works meetings.
- Additional Duties: Perform other tasks as assigned to support program success.
Qualifications & Skills
- Education: Bachelor's degree in social services or related field; Master's in Social Work, Counseling, or Psychology preferred.
- Experience: At least two years working directly with homeless individuals with disabilities, including mental health and substance use disorders.
- Knowledge: Understanding of trauma-informed care and the Housing First approach.
- Skills: Crisis intervention, managing multiple responsibilities, independently and collaboratively working with teams, excellent communication, and proficiency in Microsoft Office 365.
- Drive & Validity: Must have a clean driving record (client transportation may be required).
- Language Skills: Bilingual in English/Spanish preferred.
Physical Demands & Work Environment
Employees must be able to talk, hear, handle objects, and occasionally stand, walk, sit, or reach. lifting up to 25 pounds may be necessary. Vision, focus, and moderate noise levels are typical for this role. Reasonable accommodations are available for individuals with disabilities.
