Position Details

Note: This is a hybrid role with a minimum of two to three days on-site per week in Tukwila, WA 98168.
Job Summary
At Neighborhood House, we are dedicated to helping our community's low-income, immigrant, and refugee aging and disabled members live with independence and high quality of life. The Case Manager reports to the Community Health Supervisor and plays a critical role in developing personalized care plans, supporting clients in maintaining their activities of daily living, and navigating health and social services. A compassionate, culturally aware mindset and experience with aging and disabled populations are essential.
Key Responsibilities
- Support outreach efforts to inform the community about available services.
- Conduct in-home functional assessments for approximately 75-85 clients, creating tailored care plans.
- Review client eligibility annually and as needed to ensure ongoing support.
- Oversee and review care plans regularly to meet client goals.
- Maintain confidential, accurate documentation of all client activities.
- Provide information, referrals, and advocacy to connect clients with appropriate resources.
- Participate in clinical case reviews, training sessions, and consultations.
- Assess placement options (e.g., Assisted Living, Adult Family Homes) and plan for client transitions or exits.
- Ensure compliance with DSHS Quality Assurance and HIPAA standards.
- Travel to client homes across King County for assessments.
- Work collaboratively with interpreters as needed.
- May require evening and/or weekend work.
- Perform other duties as assigned to support team needs.
Qualifications & Requirements
Minimum Qualifications
- Bachelor’s degree in Social Work, Behavioral Science, or related field plus 3 years of relevant experience, OR
- Associate’s degree with 5 years of social work or human services experience focusing on aging and diverse populations.
Language & Cultural Competency
- Bilingual in Arabic and Kurdish, with English proficiency for documentation and online navigation.
- Ability to thrive in a diverse, equitable, and inclusive workplace. A commitment to racial equity and social justice is essential.
Experience & Skills
- Experience working with elderly or disabled adults with complex health and social needs.
- Strong understanding of federal and state regulations, and meticulous documentation skills.
- Knowledge of resources that support independence for seniors and individuals with disabilities.
- Excellent communication skills, both verbal and written, across diverse cultural backgrounds.
- Reliable transportation for home visits and worksite travel.
- Self-motivated, highly organized, dependable, with strong problem-solving skills.
- Proficient in MS Office applications (Word, Excel, Access).
