Job Summary

Our agency is dedicated to ensuring that low-income, immigrant, refugee, aging, and disabled community members can age in place with culturally appropriate services that enhance their quality of life. The primary focus of the Case Manager role is to assist clients and their families to access the supports needed to remain in their homes. This position reports to the Community Health Supervisor.

The Role

The Case Manager will develop and implement personalized care plans to help clients achieve their goals and address all Activities of Daily Living (such as cooking, cleaning, bathing, etc.).

The ideal candidate will:

  • Have experience navigating within dynamic and flexible environments
  • Work effectively with aging and disabled populations

Responsibilities

  • Support program outreach activities to inform the community about available services 🌐
  • Develop care plans tailored to individual client needs
  • Assist clients with accessing resources and support systems
  • Coordinate services to ensure comprehensive care
  • Maintain accurate documentation and case records
