ACRS is dedicated to promoting social justice and providing comprehensive behavioral health and human services to Asian, Pacific Islander, and underserved communities since 1973. We are seeking a compassionate and culturally competent full-time Case Manager I, fluent in Korean, to support elders and adults with disabilities within our Aging and Adult Services department. About the Role

As a Case Manager I (Korean Speaking), your primary responsibility will be delivering holistic, person-centered, and strengths-based case management services. You will help individuals achieve independence and meaningful community living while ensuring compliance with agency policies and regulatory standards. What We Offer

  • Competitive Salary: $58,900 to $63,700 annually, with potential increases based on experience.

Key Responsibilities

  • Develop & Implement Care Plans: Conduct comprehensive in-home assessments and create personalized, community-based care strategies in collaboration with clients, families, and healthcare providers.
  • Resource Linkages: Identify needs and connect clients with community resources, ensuring follow-up and service meeting.
  • Authorization & Monitoring: Approve services and manage provider selections based on care plans; facilitate home visits and phone check-ins.
  • Financial & Service Authorization: Manage Medicaid-funded service payments and inform clients of service changes or terminations.
  • Client Advocacy: Support clients in accessing services such as housing, medical resources, legal aid, and translation/interpretation. Assist with applications, documentation, and counseling.
  • Transitions & Safety: Coordinate placements in care settings, facilitate client transitions, and act as mandatory Adult Protective Services (APS) reporter to ensure safety.
  • Crisis Support: Provide short-term crisis intervention and determine appropriate care settings.
  • Documentation & Quality Assurance: Complete timely reports, participate in program evaluations, and ensure compliance with standards.
  • Team Participation: Attend meetings, trainings, and contribute to continuous improvement initiatives.

Qualifications

  • Education & Experience:
    • Master's degree in social work, behavioral sciences, health sciences, or related field preferred.
    • Alternatively, a Bachelor's degree with relevant bilingual or bicultural skills.
  • Language Skills: Fluency in Korean and English required.
  • Other Skills: Strong communication, cultural competence, and commitment to social justice.
