Division Overview 🌱
The Children, Youth, Family, and Transitional Age Youth division offers comprehensive early care, education, family support, and mental health services across San Francisco to more than 400 families. Using a two-generation approach rooted in the Strengthening Families Framework, we focus on five key protective factors: parental resilience, social connections, parenting and child development knowledge, concrete support, and social-emotional development in children.
Family Support Services 🧸
Our Family Resource Centers (FRC) provide wraparound services to support young parents and families citywide. Our inclusive centers serve all cultural backgrounds and family types. Key programs include:
- Intensive care management
- Support groups promoting parent leadership and family health
- Workshops on child development, health, wellness, and family engagement
- Community events
- Curriculum-based parenting workshops (e.g., STEP, PIPE, Abriendo Puertas)
- Parent advocacy, mental health support, and counseling
- Economic empowerment programs, fatherhood initiatives, GED prep, educational support, and help with benefits like CalFresh and housing
Position Overview: Bilingual Case Manager 🎯
The Bilingual Case Manager will work directly with families at our Young Family Resource Center, providing care coordination, advocacy, and resource linkage to support their well-being. Responsibilities include conducting assessments, developing individualized care plans, maintaining case records, and participating in team and network activities.
Key Responsibilities 📝
- Manage up to 50 ECE families or young pregnant/parenting teens
- Complete initial and bi-annual assessments, including care planning
- Document service delivery in Salesforce and maintain physical case files
- Develop and monitor individualized service plans, updating quarterly
- Provide intensive, wrap-around case management and referrals
- Assess physical and mental health needs to tailor care plans
- Connect families with healthcare, social services, and benefits
- Offer counseling, resource information, and interventions as needed
- Maintain regular contact through office visits, calls, and home visits (virtual included)
- Track client progress, adjust plans as necessary, and complete case documentation
- Support mental health and crisis interventions within the team
- Conduct outreach via presentations, flyers, and social media platforms
- Ensure program compliance with funder and contractual requirements
- Represent the Family Support team in community meetings and partnerships
Minimum Qualifications ✅
- At least 3 years of case management or treatment experience with high-need individuals or families
- Bachelor’s Degree in Social Work or related field
- Fluent in Spanish and English, both verbal and written
- Compassionate with excellent interpersonal skills
- Motivational approach to encourage client engagement
- Strong communication skills to maintain detailed case records
- Critical thinking and problem-solving abilities
- Organizational skills to manage multiple cases effectively
- Proficient in Microsoft Word, Excel, and Access
- Ability to handle confidential information professionally
- Team-oriented attitude with a welcoming demeanor

- Salary Range: $52,000 - $55,000 annually (DOE)
