Selfhelp Community Services Inc Is Hiring A Case Manager Bilingual
Join our team as a Dedicated In-Home Services Coordinator, where you'll play a vital role in assessing client needs, managing care plans, and ensuring clients receive the benefits and support they deserve. 🌟
Position Summary
This role involves evaluating clients' eligibility for in-home services, entitlements, and benefits, while managing ongoing case work and care plan development to promote well-being and independence.
Principal Responsibilities
- Conduct intakes: Provide accurate information and referrals to clients.
- Assist clients: Screen and facilitate applications and re-certifications for entitlements and benefits.
- Assess clients: Conduct evaluations in home, hospital, or other institutional settings as appropriate.
- Advocate & coordinate: Arrange services, develop, implement, and follow up on care plans.
- Provide counseling: Offer supportive guidance to clients as needed.
- Maintain communication: Keep ongoing contact with clients, family members, and other stakeholders.
- Documentation: Keep thorough case notes using the program’s data system and prepare required reports within organizational deadlines.
- Reporting: Track and report statistics in accordance with Selfhelp standards and data management protocols.
- Team participation: Engage in supervision, staff meetings, and share emergent issues promptly.
- Plan activities: Facilitate social, recreational, and group events to support client engagement.
- Additional duties: Perform any other responsibilities as assigned.
- Organization standards: Complete annual mandatory trainings on topics like Sexual Harassment Prevention, IT Security, Compliance, OSHA, and HIPAA in a timely manner.
- Support mission & values: Strive for excellence and foster positive relationships with colleagues.
- Policy adherence: Follow policies related to attendance, appearance, health, and safety.
Salary Range 💰
$54,000 - $56,400 per year, commensurate with experience.
Job Competencies & Minimum Qualifications
- Educational Requirement: Bachelor's Degree is required.
- Language Skills: Fluency in Russian and English is mandatory.
- Experience: Minimum of one year related experience and/or training.
- Technical Skills: Working knowledge of Microsoft Office Suite and other relevant technologies.
- Communication: Excellent verbal and listening skills.
- Customer Service: Ability to treat clients with respect and dignity.
- Multitasking & Independence: Capable of managing multiple tasks with great attention to detail.
Working Conditions & Physical Demands 🚶♀️🏢
This position is primarily in a business office environment involving phone and computer use, with travel via public transportation. There is potential to work from home 2-3 days per week.
