Position: Case Manager (Bilingual: English & Spanish or Portuguese)

Location: Brockton

Job Summary 🧩

The clinical goal of this program is to provide youth under 21 and their families with short-term, intensive treatment to help them stay safely at home and connect with ongoing outpatient or community-based services. The FIT service includes Intensive Family Therapy, which may involve entire families or subsets thereof, utilizing targeted, evidence-based interventions to improve communication, problem-solving, safety planning, and overall family functioning.

Key Duties & Responsibilities 📝

  • Support treatment plan implementation under clinician supervision to help youth and families achieve their goals
  • Teach youth emotion management and coping skills, and assist families with mental health needs
  • Conduct phone consultations and provide crisis/emergency support 📞🚑
  • Develop and review safety and risk management plans collaboratively with families
  • Provide or coordinate emergency services as needed
  • Engage with involved agencies, participate in Care Planning Team meetings, and maintain communication 🏢
  • Complete all required documentation, including court letters, referrals, progress notes, and forms with a strength-based approach ✍️
  • Ensure timely submission of timesheets, mileage logs, and related paperwork
  • Stay current with all training and health requirements (e.g., OSHA, Sexual Harassment)

Minimum Qualifications ✅

  • Bachelor’s degree in a human service field (counseling, social work, psychology, etc.) from an accredited university
  • Experience in family therapy and working with youth and families
  • Experience navigating child/family-serving systems and advocating for clients
  • Demonstrated creativity, problem-solving, and organizational skills
  • Ability to work effectively with diverse populations and community agencies such as schools, DYS, and juvenile courts
  • Willingness to support youth and families 24/7 throughout the year
  • Valid driver’s license and reliable vehicle to support program needs 🚗
