🔍 Position Overview

The bilingual Family Support Case Worker will develop individual child case plans, monitor progress, and maintain comprehensive case files. This includes providing information, education, referrals, outreach, advocacy, and support to facilitate the post-release home placement of children. The role involves building a network of contacts and referral resources to aid sponsors and children, assessing safety and appropriate placement, and supporting family unification, family preservation, kinship care, and overall child well-being in the Juntos home study program.

📝 Duties and Responsibilities

  • Provide information, education, referrals, outreach, advocacy, and support to ensure each child's needs are met.
  • Assess the safety and suitability of each placement continually using child welfare principles and social work best practices.
  • Develop individualized case plans with attention to culture, language, and special circumstances.
  • Maintain accurate case files by updating databases, submitting reports, and managing physical files.
  • Build a network of local resources including legal aid, food and clothing banks, mental health services, and care providers.
  • Track individual progress via an online database.
  • Coordinate and maintain communication with key stakeholders.
  • Participate in regular training sessions with staff and supervisors.
  • Maintain confidentiality in all service-related areas.
  • Uphold professional and ethical standards as outlined in the employee handbook.
  • Meet or exceed productivity expectations consistently.
  • Perform services remotely from home as needed.
  • Complete other duties as assigned.

🎓 Requirements

  • Bachelor's Degree in Social Work (BSW) or an equivalent in education, psychology, sociology, or behavioral sciences.
  • Bilingual in English and Spanish.
