Position Overview
Salary Range: $43,084 - $56,547.75
This is an exempt ( salaried ) position. Pay rates are determined based on education, skills, experience level, and internal equity considerations, including salary history and job market requirements.
Note: This role is on-site 5 days a week.

Position Specific Information
About the Division of Child and Family Well-Being 🧸
The NC DHHS Division of Child and Family Well-Being (DCFW) champions whole child and family health, aiming to ensure children grow up safe, healthy, and thriving within nurturing communities. DCFW collaborates with divisions like Public Health, Mental Health, and Social Services to maximize service outcomes, including nutrition programs (SNAP, WIC, Child and Adult Care Food Program), behavioral health services, and early intervention initiatives.
The North Carolina Early Intervention Section (NCEI), part of DCFW, leads the Infant-Toddler Program (ITP)—offering vital supports for families and children aged birth to three with developmental delays or at risk. This critical period provides a unique opportunity to positively influence a child's growth and learning. Numerous CDSAs across NC partner with local providers to support family success.
Position Summary 🎯
The Case Coordinator plays a key role in providing professional case management services for the infant-toddler program. As Mecklenburg County’s local agency for Early Intervention, CDSA helps families foster children's development during these formative years. Under supervision, the coordinator contributes to treatment planning and develops person-centered plans tailored to each family's needs.
Essential Functions 📝
- Review cases and provide supporting documentation for clients receiving financial assistance
- Interview and screen potential clients; review mental health/substance abuse history; consult with relevant individuals; coordinate discharge planning
- Evaluate clients' immediate circumstances, including environmental factors and available services, to develop effective service plans
- Prepare reports related to client progress and ensure compliance with state policies and procedures
- Foster inter-agency collaborations; research additional services or programs for clients
- Coordinate new referrals and eligibility assessments for children aged birth to three in the NC Infant-Toddler Program
- Develop and implement Individualized Family Service Plans (IFSPs) with families to address their concerns, needs, and priorities
- Link and coordinate services to meet family and child needs
- Maintain confidentiality and parental rights according to procedural safeguards
- Maintain and review client progress notes via computerized charts
- Monitor client progress through home visits, community visits, or phone contacts
- Assist families in resolving concerns related to community resources, treatment, or social issues—supporting coping with mental health challenges
- Potentially staff shelters during disaster events
Minimum Qualifications 🔑
- Experience: Minimum of two years in applied settings such as developmental disabilities, mental health, early childhood, or related health conditions
- Education: Bachelor's degree in a human services-related field
- Combination of relevant education and experience may be accepted
Licenses & Certifications ✅
- Possession or eligibility for the NC Infant and Toddler Specialist Credential (preferred)
- Valid North Carolina or South Carolina Driver's License
- County Driving Privileges required
- Annual Motor Vehicle Record (MVR) check is mandatory
