Position Overview

As a Counselor, your role will involve providing outpatient substance use treatment to diverse clients, including adults facing issues such as homelessness, justice involvement, unemployment, and more. Your expertise will contribute to improving clients' lives through compassionate, comprehensive care.

• 🖥️ EHR documentation skills

Responsibilities and Duties

• 🧑‍🤝‍🧑 Provide care to adults with issues related to substance use, psychosocial needs, and functional impairments.
• 📋 Assist in implementing program policies, mission, and procedures.
• 📂 Maintain a caseload, conduct assessments, offer counseling, referrals, and intervention services, and perform scheduled tasks.
• 🛑 Intervene with patients regarding specific problems as needed.
• 👥 Facilitate intake and orientation sessions for new patients.
• 📝 Maintain accurate, up-to-date clinical documentation complying with regulations.
• 💡 Contribute to Performance Improvement initiatives focusing on quality of care and clinical service planning.
• 🚨 Provide crisis intervention to patients and/or families effectively.
• 🎯 Assist in setting and achieving program goals.
• 🌐 Conduct outreach to community agencies and engage absent patients.
• 🤝 Coordinate services with relevant agencies and refer patients as needed.
• 🩺 Follow up with psychiatrists and physicians when appropriate.
• 🛠️ Utilize validated tools for screening and assessing depression, trauma, substance use, and risk factors.
• 📈 Evaluate service utilization and patient outcomes continuously.
• 🌍 Deliver culturally competent services in line with Hands On Health policies.
• 🎨 Develop and facilitate group therapy curricula, either independently or collaboratively.
• 🤝 Create recovery-oriented treatment plans in collaboration with clients and stakeholders such as family members, criminal justice personnel, healthcare providers, and other agencies.

• Location: Brooklyn, New York
• Hours per week: Monday-Friday, 8am-4pm
