Responsibilities and Duties

• Provide outpatient care to adults with diverse issues such as substance use, functional impairments, psychosocial needs (e.g., homelessness, justice involvement, unemployment)
• Implement program policies, mission, and procedures
• Manage caseloads by conducting substance use assessments, counseling, referrals, and intervention services
• Intervene as needed with patients facing special problems
• Facilitate intake and orientation of new patients
• Maintain accurate clinical records in compliance with regulations
• Contribute to Performance Improvement initiatives related to quality of care and service planning
• Provide crisis intervention to patients and/or families
• Assist in developing and implementing program goals
• Conduct outreach to community agencies and locate absent patients
• Coordinate services with relevant agencies, referring patients as needed
• Follow up with psychiatrists and physicians as appropriate
• Utilize validated tools for screening and assessing depression, trauma, substance use, and relapse risk
• Evaluate services and outcomes for ongoing patient progress
• Deliver culturally competent services aligned with Hands On Health policies
• Develop and facilitate group psychotherapy sessions or co-facilitate as needed
• Create recovery-oriented treatment plans with client and stakeholder collaboration, including criminal justice and healthcare partners

Hours per Week: Monday to Friday, 8:00 am – 4:00 pm
