About On Lok PACE 🌟
We focus on the comprehensive care of seniors, especially those facing chronic health challenges. The PACE (Program of All-Inclusive Care for the Elderly) model emphasizes serving seniors in their communities to promote well-being and independence.
Seniors, their families, and caregivers encounter many challenges such as transportation, medication management, coordinating medical care with various specialists, social isolation, and remaining in their homes safely. 🏡
Participants receive in-home care, transportation to our centers for medical and social activities, and a variety of senior services. We believe that a collaborative, systems-based approach is key to effective care.

Position Overview 📝
Caregiver: Provide personal care services to On Lok participants receiving home care and/or attending our day centers.
Key Duties & Responsibilities 🎯
- Personal Care & Escort Assistance: Assist with grooming, dressing, peri-care, bathing, toileting, housekeeping, meal prep, feeding, and laundry based on tailored care plans. Report any changes in participant condition to the interdisciplinary team.
- Safe Practice Implementation: Use proper body mechanics, follow universal precautions, and ensure a safe environment.
- Participant & Family Engagement: Interact professionally, encouraging participation in conversations and activities.
- Training & Mentorship: Provide onboarding and ongoing training for new Caregivers using manuals and formal training materials.
- Team Participation: Attend departmental and family meetings as needed.
- Additional Responsibilities: Perform other related duties as assigned.
Qualifications ✨
- At least one year of experience working with frail or elderly individuals.
- Knowledge or research into the PACE model.
- Flexible with a positive attitude to work across various settings.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Proficient in using and explaining safety equipment for lifting; demonstrate positive correction techniques ("Let me help you and show you how this works").
- Physical Ability: Capable of stooping, reaching, standing, walking, pushing, pulling, lifting, grasping, talking, hearing, and repetitive wrist/hand motions. Visual acuity required. Must lift/move within your physical limits and request assistance when needed.
- Basic English reading and writing skills; bilingual Cantonese required.
- Basic computer skills: familiar with or willing to learn applications such as email and electronic health records.
