Role DescriptionThis is a full-time role as a Care Manager Supervisor at Serenity Community Care. In this role, you will be responsible for overseeing and supervising a team of care managers. You will work closely with the leadership team to implement policies and create procedures to boost your team's productivity and improve the quality of their care. You will provide guidance, support, and training to the care managers to ensure high-quality service coordination and patient and family support.

Qualifications

• Master's degree in a relevant field (e.g. social work, nursing, psychology)
• Experience with training/mentoring care managers
• Experience with Health Home Care Management
• Experience in a leadership/supervisory capacity
• Understanding of medicine, SDoH, and healthcare systems
• Strong leadership and supervisory skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively and adapt to changing situations
• Bilingual or multilingual proficiency is a plus (Yiddish, Hebrew, Spanish preferred)
