Salary Range: $29.87 - $31.93 Hourly

Overview 📝

The Care Manager plays a vital role within our Care Management programs, including Health Home Care, Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management services. Support may extend to Willowbrook Class Members as needed.

The core responsibility involves overseeing and coordinating access to services for individuals with intellectual and developmental disabilities. This involves working closely with members, their families, and providers to develop, implement, and monitor personalized, person-centered Life Plans based on comprehensive assessments.

We are committed to serving over 25,000 people across New York City, Long Island, and the Lower Hudson Valley by providing comprehensive care management and coordinated services tailored to individuals with intellectual and developmental disabilities.

Key Duties & Responsibilities 🛠️

  • Care Delivery: Provide person-centered care management aligned with regulatory standards, agency policies, and quality procedures.
  • Assessments: Complete comprehensive assessments and reassessments to identify service gaps and facilitate appropriate referrals.
  • Advocacy: Support members in achieving their goals and living fulfilling lives through advocacy and resource navigation.
  • Life Plan Development: Create, implement, and monitor individualized Life Plans with an interdisciplinary team, keeping the member at the center.
  • Conflict Resolution: Address and resolve conflicts within the planning process promptly and effectively.
  • Documentation & Billing: Ensure timely and accurate service documentation and billing compliance.
  • Eligibility & Benefits: Maintain member eligibility, including Level of Care (Re)Determinations, OPWDD eligibility, and benefit access (Medicaid, Social Security, SNAP).
  • Consent & Information Sharing: Manage current consent documentation within electronic health records, updating as needed.
  • Healthcare Coordination: Facilitate access to medical, behavioral health, and specialized services, ensuring continuous communication and follow-up.
  • Preventive & Transitional Care: Coordinate health promotion activities, inpatient follow-ups, discharge planning, and transitions within healthcare and social systems.
  • Health Technology Use: Utilize electronic health records and telehealth platforms effectively, maintaining comprehensive member records.
  • Team Engagement & Training: Participate in meetings, trainings, and supervision consistent with agency policies; complete required training modules.
  • Travel & Outreach: Travel within the service area to meet members, providers, and teams, and support appointments as needed.
  • Incident & Confidentiality: Follow reporting procedures for incidents, maintaining confidentiality per HIPAA and privacy policies.
  • Policy Compliance: Uphold all organizational policies, including the Code of Conduct, and perform additional duties as assigned.

Qualifications 🎓

  • Bachelor’s degree (Arts or Science) with at least two years of relevant experience, or a licensed Registered Nurse with similar experience, or a master's degree with one year of relevant experience.
  • Fluent understanding of community cultural traditions, norms, and practices of members and their families.
  • Strong integrity and personal commitment to the mission and values of ACA/NY.
