Care Design New York Care Manager

As a Care Manager, you will develop, implement, and monitor personalized Life Plans that empower members to live their best lives. Your relational and organizational skills will play a vital role in delivering exceptional support. All Care Managers receive comprehensive training to excel in their profession.

This position offers a flexible, hybrid work environment involving member visits, training sessions, and remote work. If supporting members outside NYC, a valid driver’s license, reliable transportation, and appropriate insurance are required.

Responsibilities

• Advocate: Promote informed decision-making, consent, and guardianship for members.
• Collaborate: Lead and participate in meetings with members and interdisciplinary teams to create comprehensive, person-centered Life Plans that reflect individual needs and goals.
• Implement & Monitor: Develop, update, and review Life Plans, ensuring integration of necessary supports and services.
• Maintain Contact: Keep ongoing communication with key individuals in a member's life.
• Documentation: Ensure timely submission of all required documentation such as Life Plans and progress notes.
• Benefits Management: Assist members with maintaining Social Security, SSI, Medicaid, Medicare, and Food Stamps.
• Problem Resolution: Support members in resolving issues related to housing, utilities, legal matters, and safety.
• Report: Immediately report any observed or reported abuse or neglect.

Qualifications

• Education & Experience:
  • Bachelor's degree with 2+ years of relevant experience
  • Or a Licensed Registered Nurse with 2+ years of experience
  • Or a Master's degree with 1+ year of relevant experience
• Skills: Excellent verbal and written communication skills, along with strong interpersonal and organizational abilities.

Salary Range

Starting at: $27.75 per hour

Up to: $33.49 per hour
