Position: Care Manager (Mandarin, Hindi, Punjabi, Urdu preferred)
Overview
As a Care Design New York Care Manager, you will assist individuals with disabilities in identifying and reaching their short-term and long-term goals through the development, implementation, and monitoring of person-centered Life Plans. These plans empower our members to live their best lives, relying on your exceptional relational and organizational skills. Our Care Managers come from diverse backgrounds, and all receive top-tier training in the profession.
Bring your passion for service into our care model to deliver the highest standard of support to our members. This dynamic role involves a hybrid work environment, including visits to members' homes, attending office trainings and meetings, and working remotely. If supporting members outside NYC, a driver’s license, reliable transportation, and appropriate insurance are required.

Responsibilities
- Advocate for and with members to ensure informed decision-making, consent, and guardianship.
- Schedule and lead meetings with members and interdisciplinary teams to develop comprehensive, person-centered Life Plans aligned with individual needs and goals.
- Implement, update, and monitor Life Plans, facilitating reviews and approvals.
- Integrate all necessary supports such as medical, behavioral, social, habilitation, dental, psychosocial, community-based, and facility-based services.
- Maintain ongoing contact with key people in a member’s life.
- Ensure timely submission of documentation like Life Plans and progress notes.
- Assist members in maintaining benefits like Social Security, Medicaid, Medicare, and Food Stamps.
- Support members in resolving living challenges related to housing, utilities, safety, and legal matters.
- Report any abuse or neglect immediately.
- Provide a detailed job description during interviews.
Qualifications
- Bachelor’s degree with 2+ years of relevant experience, or
- Registered Nurse license with 2+ years of relevant experience, which can include various employment types, or
- Master’s degree with 1+ year of relevant experience

Salary
Hourly Rate: $27.75 – $33.49
