About the Role

As a Care Manager, you will be instrumental in helping members develop and follow personalized Life Plans that promote independence, fulfillment, and well-being. Your relational and organizational skills will be key to delivering top-quality service in a compassionate, person-centered manner. 💙

Work Environment & Compensation

This role operates within a hybrid setting that includes:

  • Visits to members' homes in NYC and beyond 🚗
  • Periodic training sessions and meetings at the office 🏢
  • Remote work opportunities from home 💻

Applicants supporting members outside of New York City should possess a valid driver’s license, reliable transportation, and appropriate insurance.

We offer competitive pay: $27.75 to $30.19 per hour plus robust benefits:

Key Responsibilities

  • Advocate for members to ensure informed decision-making and appropriate guardianship 🤝
  • Schedule, lead, and collaborate with members and interdisciplinary teams to develop comprehensive Life Plans 📝
  • Implement, monitor, and review Life Plans, facilitating adjustments as needed 🔄
  • Coordinate all necessary supports and services—medical, behavioral, social, and community-based 🏥
  • Maintain ongoing contact with critical individuals in a member’s life 📞
  • Ensure timely submission of documentation such as Life Plans and progress notes 🗂️
  • Assist members with benefits management, including Social Security, Medicaid, and food assistance programs 🛡️
  • Help resolve living and safety issues related to housing, utilities, and legal concerns 🏡⚖️
  • Report any abuse or neglect immediately 🚨

Qualifications

  • Bachelor’s degree with 2+ years of relevant experience, OR
  • Registered Nurse license with 2+ years of relevant experience, including diverse employment backgrounds, OR
  • Master’s degree with 1+ year of relevant experience 🎓
