Job Overview

The Care Manager collaborates closely with individuals, their families, OPWDD, and service providers to ensure they live safe, happy, and fulfilling lives. 🌟 Responsibilities include identifying personal goals, developing tailored plans to achieve them, and assisting with access to supports and services such as social security, SNAP benefits, and waiver programs. Our programs support members of all ages, including advocating in educational settings and medical appointments.

Location & Travel Requirements 🚗

  • This position is remote but requires occasional in-person visits.
  • Applicants must be able to travel via car or public transportation to these visits.
  • The caseload will be based in or near the designated county, but not limited to it.
  • Applicants outside New York City must hold a valid driver’s license and have reliable transportation.
  • Travel reimbursement is provided.
  • The work environment should be quiet and distraction-free, or applicants should be able to work from one of our regional offices.

Requirements ✅

Language Skills

  • Fluent in English & Spanish

Knowledge, Skills, and Abilities

  • Excellent communication skills (both written and verbal)
  • Conflict management and problem-solving capabilities
  • Proficiency with Microsoft Office programs (Word, Excel, Outlook)
  • Strong organizational and multitasking skills
  • Ability to analyze situations, prioritize, and take effective action

Education, Experience, and Licenses

  • Bachelor’s degree with two years of relevant experience, OR
  • Registered Nurse license with two years of relevant experience (including various employment experiences, not limited to case management), OR
  • Master’s degree with one year of relevant experience
  • MSC Service Coordinators prior to July 1, 2018, are “grandfathered” to ensure continuity of care
