Join Our Team as a Resource Coordinator (Care Manager)! 🚀

As a Resource Coordinator (Care Manager), you'll play a vital role in outreach, enrollment, and providing guidance to individuals, families, and caregivers about available services. You will work as part of an interdisciplinary team to coordinate medical, social, and behavioral health needs, ensuring families access necessary resources to stay healthy, prevent hospitalizations, and maintain stability.

Note: Home visits are required in the Queens or Long Island areas.

Key Responsibilities ✔️

  • Conduct recruitment activities and actively monitor the waiting list to ensure accurate placement of eligible families and individuals.
  • Complete intake forms, enrollments, and ongoing assessments to meet client needs.
  • Facilitate interdisciplinary collaboration among providers, enrollees, families, caregivers, and community supports.
  • Ensure families are aware of support services and entitlements, providing recommendations and referrals to community resources.
  • Advocate on behalf of individuals and families, serving as a liaison between them and other programs.
  • Perform in-office, home, and community visits to assess needs and deliver services accordingly.
  • Engage in outreach efforts within the community to identify potential program candidates.
  • Coordinate services for families with children with disabilities or special needs, offering targeted support.
  • Maintain compliance with all Federal, State, and Local regulations, utilizing data management systems to monitor adherence.

Minimum Qualifications 📋

  • Bachelor’s degree in Human Services or a related field
  • At least 2 years of experience in Human Services, Psychology, or a similar field, including coursework in social work, child development, counseling, or psychology
  • Ability to plan and execute assignments independently
  • Strong prioritization, time management, and multitasking skills
  • Home visits are required
