CARES Is Hiring A Care Manager Bilingual Russian

Position: Care Manager (Bilingual Russian)

Salary Range: Fee for Service range of $100-$300 per case, approximately $55,000-$65,000 annually

Location & Travel

  • Dyker Heights office, Brooklyn, NY 🏢
  • Travel throughout the 5 Boroughs, Orange and Sullivan Counties 🚗

Essential Duties and Responsibilities

  • Coordinate services for children and families: Manage overall care plans to meet clients’ goals by building strong relationships with caregivers and care teams.
  • Act as a liaison: Collaborate with other agencies to ensure clients receive all necessary services and implement individualized care plans.
  • Maintain compliance and quality: Deliver top-tier care management services aligned with regulatory guidelines and agency standards.
  • Work with Medicaid-insured children: Ensure access to proper medical and emotional support, facilitating service access on a monthly basis.
  • Assessments & Care Planning: Complete assessments to identify medical, social, educational, and psychological needs; develop comprehensive care plans based on these needs.
  • Service Coordination: Monitor delivery of services and resource utilization to optimize care quality.
  • Monthly Client Services: Deliver at least 2 client-centered health home services per month.
  • Professional Development: Attend training sessions, staff in-services, and quarterly meetings.
  • Quality Assurance: Respond promptly to feedback and implement corrections as needed.
  • Additional Duties: Perform related tasks assigned by supervisors and maintain company policies and procedures.

Education & Work Experience

  • Required: Bachelor's degree in Psychology, Sociology, Social Work, Human Services, or related field OR at least 2 years of relevant case management or care coordination experience.
  • Ability to attend in-person trainings and meetings at our Midwood, Brooklyn office approximately once per quarter.
  • Conduct monthly in-home visits; hybrid work model possible after managing a full caseload (~20 cases).
  • Skills: Excellent communication (verbal and written), organizational skills, computer proficiency (MS Office: Word, Excel, Outlook), and experience with electronic health records.
  • Language Skills: Bilingual in Russian.
  • Driver’s License: Valid NYS driver’s license and willingness to travel across Orange and Sullivan Counties.
  • Ability to work independently and complete assignments within prescribed routines and standards.

Physical & Travel Requirements

  • Ability to travel to client locations, healthcare facilities, and attend meetings.
  • Safely perform essential job functions in accordance with ADA, FMLA, and other relevant standards.
  • Must be able to lift and carry up to 35 lbs.
  • Excellent verbal communication skills to effectively talk, listen, and speak clearly.
